Player Accident Insurance

North Sydney Futsal has a player accident & injury policy for players registered with teams involved in regular games in the competition.

If you are injured in our games please email admin@northsydneyfutsal.com with your name, team, date and outlining details of your injury. Please report ALL injuries immediately or on the day after your game. There may be time limits for reporting injuries or submitting a claim to the insurance company.

Procedure

1) Report the injury on the night of your game to the administrator or immediately after via email.

2) Email admin@northsydneyfutsal.com for claim forms. Follow instructions as provided by the insurance company. If you need any advice or have any questions please ask our insurance broker.
